If using a desktop your download will appear on lower left corner. If using a phone look in your downloaded files.
Click download, open file , enable editing and fill it out.
Once you have completed application please attach and email it to lovingpawsadoptioncenter@gmail.com.
Please be sure to contact your vet and give permission for a Loving Paws /Pulaski County Humane Society representative to call and inquire about your pet(s).
